  Patients with end-stage kidney disease often have multiple comorbid conditions, such as diabetes, hypertension and cardiovascular disease. They must navigate a multitude of specialty doctor appointments, medications and lifestyle needs—all of this on top of their day-to-day worries about their health. This puts them at a higher risk of care fragmentation, which is associated with reduced quality of care, increased costs, and poor clinical outcomes. Together with BJC HealthCare, we created the Shared Patient Care Coordination (SPCC) program, a novel program aimed at reducing care fragmentation. In its first year, SPCC resulted in better patient outcomes with a reduction in hospitalizations and missed treatments.
